For your convenience, applications for admissions to Brownwood Life Care Center can be completed online. Even though some fields do not need to be completed to submit the application, the information will need to be provided to Brownwood before your child can be admitted to our facility.
Applications can also be obtained by downloading the PDF below and printing it, or you may pick up an application at 7500 Wells Lake Road, Fort Smith, Arkansas.
Once a completed application is returned, the Interdisciplinary Team will evaluate the information to determine whether a child’s needs can be met at our facility. Records that are required in order for a client to be considered for admission may include past medical, education, and therapy history.
